Exclusion and Limitations

NOT ACCEPTED: Care Source, Medicaid, Superior Vision, Medical Mutual, Medicare, Community Plans, United Health Care (UHC), Molina, Guardian, Ameritas, Vision Benefits of America (VBA), Avesis, WellPoint, Community Eye Care (CES), Advantica, Aflac, or other plans.

Some vision benefits plans are only accepted as “out of network” or “discount” plans. If you have a plan that we do not accept, we will provide itemized receipt in case that out of network benefits are available.

Vision Insurance is NOT ACCEPTED on CARTIER, MAUI JIM, and DITA Eyewear and lenses.
